1. Install the game using either the retail, gog or steam version and make sure it is up to date. Then run it once in order to create any user config files needed. you should also take this time to configure the graphics settings as you see fit.
2. Install Nexus Mod Manager (NMM) and run it once. It will require you to perform a first time setup and log in. Refer to the "Important Software" page linked at the top.

NOTE.
It is important to backup your game files somewhere in case something goes wrong, then you can just delete and start again without reinstalling. this means your game install files and also your saves/config files etc.

Sometimes two mods will make changes to the same script. this will result in the second mod breaking the first mod. To fix this issue the most excellent Script Merger, by AnotherSymbiote, will merge two conflicting scripts into one to preserve functionality.

It is important that you check for new conflicts after every mod you install otherwise at the end of this you will have dozens of conflicts to resolve. I am never going to mention this again as I assume you will be doing this. Disable the intro.
OK the intro vids and storybooks are great at first but during modding this game you'll be restarting A LOT and so it saves considerable time to disable all intros at least temporarily. To do this go to:

and rename the file "bumpers.usm" to something else like "AAbumpers.usm"
Then install Disable Storybook Videos using NMM. to uninstall rename the file back to default and disable the mod in NMM.

Make mods compatible across game versions.
The Unification patch for 1.31 and 1.31GOTY game versions mod makes all mods for the GOTY version backwards compatible for other versions of the game. Pay careful attention to these instructions:

TW3's HUD can be modified through the in game menu, however there are still improvements to be made to text, the map and HUD functionality and even adding new graphics options to the menu.

This mod will add a multitude of advanced graphics options to the menu. Be warned that it can enable settings that will cripple any machine, settings so high that there is barely a perceptible difference in game, so experiment with the settings one at a time if you are unsure of what you are doing. Yeah I got sick of the guards being so smug knowing that the game code prevents Geralt from kicking their asses. This mod makes the generic guards in the game beatable (Geralt will knock them out). there are three 'difficulty' levels available, the main file is the best option I felt. Install with the NMM.

This mod makes food and drink heal over time. I like this mod because it has an alternate version that makes food and drink heal over hours, so it makes food and drink seem far more useful and realistic.

The vanilla trophies were a bit of a letdown, the stat boosts were insignificant and that made them largely ornamental. One thing I hated with vanilla TW3 was that I could physically acquire a sword in game but couldn't use it because my level was too low. This mod removes restrictions so now going after high level targets for high level loot becomes a worthwhile challenge. For some reason you have to install this manually.

Visual enhancements.
To be honest browsing the various "HD texture" mods and new character models I came to the conclusion that the in game textures and vanilla characters were as good as the mod versions, so I chose to save myself the trouble of installing these. Honorable mentions that you may wish to install that I didn't include are The Witcher 3 HD Reworked Project and COMPILATION.

To install use NMM to install the main file, then go to "The Witcher 3 Wild Hunt\Mods\modSwordFX" directory and move the modswordfx.xml file to the "The Witcher 3 Wild Hunt\bin\config\r4game\user_config_matrix\pc" directory.
